Appointment Cancellations
Appointments - In the event that an appointment needs to be canceled, a 48 Business-hour notice is required. Please be advised that our office is not open on Weekends or Major Holidays. (For example, if you have an appointment on Monday, you must cancel by Friday to avoid a penalty).
No Show Fees
Cancellations less than 48 business hours and no-shows will be subject to a $50 cancellation fee for follow-up appointments and a $100 cancellation fee for Annual Physical exam appointments.
APPOINTMENT REMINDERS AND TIMELINESS
Our practice makes every effort to run on time with appointments, as we believe everyone’s time is equally valuable. As a courtesy, we will remind you of your upcoming appointments via text/phone/email.
NOTE:- You will typically receive 2 to 3 TEXT reminders. TEXT Messaging is usually faster and more convenient.
Masks are Not required at our offices for all, but Only if you are above age 70 and if you have a cough or Flu.
Add our contact information to your phone address book so that it gets delivered without issues. Download the contact card to your address book
You should arrive 15 minutes before your scheduled appointment time. We understand sometimes things happen beyond your control that may cause you to be late. However, we reserve the right to ask you to reschedule if you arrive late for your appointment.
INSURANCE AND BILLING
Make sure we participate with your insurance plan. Click here for a list of our participating insurances. Our billing staff will do their best to assist you with insurance questions; however, if you have questions about your coverage, it is best check with your specific insurance company. It is your responsibility to know the limits and coverage of your particular health insurance policy, to show your cards to us at each visit, and be prepared to pay any copays at the time of service. Some referrals require pre-authorization by the insurance company and it will be your responsibility. We do Not do pre-authorizations for you.
Credit Card on File: We require all our patients to have a credit card on file. There are no exceptions.
If your insurance has changed, before your next visit, please fill out our Insurance Update Form and bring it in with your new insurance card. Insurance copays are expected to be paid at the time of service.
OFFICE HOURS AND AFTER HOURS
Your health is our PRIMARY concern!
To ensure we can provide you with the most efficient care we recommend you book online or call ahead to schedule an appointment.
We have a provider on beeper call for non-emergencies 24/7/365. If it's an Emergency you should call 911. When our office is closed overnight and you have an urgent issue that cannot wait for office hours, you can call our main number and the answering service will connect you to the provider on-call who will give you advice regarding your problem. These are not meant for non-urgent issues, and you may be asked to visit us at the clinic for non-urgent issues.
